Unit Overview

Description

The unit will provide an understanding of people living in residential aged care facilities to prepare pharmacists to work as pharmacists in aged care. People who have completed this unit will be credentialled to work as aged care onsite pharmacists in residential aged care facilities.

Learners will use evidence based materials to develop the the necessary clinical knowledge to work in RACFs. Principles of geriatric medication management will be covered including evidence-based approaches to undertake interventions to optimise medicines including deprescribing and medicines simplification.

Learners will be given practical training and tools to implement organisational change and undertake quality improvement activities. The Towards Organisational Culture Change (TOrCCh) Toolkit will be used as an evidence-based approach to support learners to undertake changes in the workplace linked to a quality improvement cycle.

Leveraging the Towards Organisational Culture Change (TOrCCh) Toolkit, participants will gain a systematic approach to effecting positive changes in workplace practices and undertake quality improvement activities to enhance resident care outcomes.

Outcomes

Students are able to (1) synthesis of the investigation, diagnosis and management of health conditions for people in residential aged care facilties; (2) describe the governance and process structures that apply in residential aged care facilties; and (3) design interventions to improve quality use of medicines in residential aged care facilities.

Assessment

Indicative assessments in this unit are as follows: (1) community engagement activity; (2) clinical quality improvement activity; (3) case studies; and (4) completion of elearning modules/activities. Further information is available in the unit outline.

To pass this unit, a student must: (a) achieve an overall mark of 50 per cent or higher for the unit; and (b) achieve the requisite requirements(s) or a mark of 50 per cent or greater, whichever is higher and specified in the unit outline, for the community engagement activity, case studies, and completion of elearning modules/activities components.

Student may be offered supplementary assessment in this unit if they meet the eligibility criteria.

Professional Recognition
This program is seeking, but has not yet been granted, accreditation from the Australian Pharmacy Council.

Requirements for enrolment:
- All students must be generally registered pharmacists with AHPRA.
- Provisionally registered pharmacists registered with AHPRA must provide evidence of support from the preceptor. The preceptor must be able to demonstrate that they are a registered pharmacist credentialled for medication management reviews.
- All students must have vaccination training prior to commencing the unit or be concurrently completing their vaccination training.

Students will be required to attend 6 synchronous tutorials at scheduled times and to undertake in person work integrated learning.

Preparatory activites required at the commencement of this unit include:
- UWA Clinical Yarning
- Dementia Training Australia's polypharmacy and deprescribing modules
- Vaccination training, completed or concurrent completion.
- Evidence of AHPRA registration as a generally registered practicing pharmacist
