Unit Overview
- Description
The unit will provide an understanding of people living with complex health needs (e.g. multimorbidity) for an understanding of the psycho-social, investigative and management options.
This unit will prepare learners to undertake Medication Management Reviews for people with complex care needs. This program is seeking, but has not yet been granted, accreditation from the Australian Pharmacy Council

- Outcomes
Students are able to (1) identify relevant information required to assess medication regimens for people with complex health needs; (2) demonstrate an understanding of common health conditions in older people; and (3) assessment of medication regimens for people with complex health needs.
- Assessment
Indicative assessments in this unit are as follows: (1) completion certificates from completion of online training modules; (2) case studies (each case study assessed as an equal proportion of the mark); and (3) discussion board interactions. Further information is available in the unit outline.
To pass this unit, a student must: (a) achieve an overall mark of 50 per cent or higher for the unit; and (b) achieve the requisite requirements(s) or a mark of 50 per cent or greater, whichever is higher and specified in the unit outline, for the completion certificates from completion of online training modules component.
Student may be offered supplementary assessment in this unit if they meet the eligibility criteria.

This program is seeking, but has not yet been granted, accreditation from the Australian Pharmacy Council.
Requirements for enrolment:
- All students must be generally registered pharmacists with AHPRA.
- Provisionally registered pharmacists registered with AHPRA must provide evidence of support from the preceptor. The preceptor must be able to demonstrate that they are a registered pharmacist credentialled for medication management reviews.
- All students must have vaccination training prior to commencing the unit or be concurrently completing their vaccination training.
Students will be required to attend 6 synchronous tutorials at scheduled times and to undertake in person work integrated learning.
Preparatory activites required at the commencement of this unit include:
- UWA Clinical Yarning
- Dementia Training Australia's polypharmacy and deprescribing modules
- Vaccination training, completed or concurrent completion.
- Evidence of AHPRA registration as a generally registered practicing pharmacist

100% Online Unit. NO campus face-to-face attendance is required to complete this unit. All study requirements are online only. Unit is asynchronous delivery, with NO requirement for students to participate online at specific times.

100% Online Unit. NO campus face-to-face attendance is required to complete this unit. All study requirements are online only. Unit includes some synchronous components, with a requirement for students to participate online at specific times.
Online Restricted

NO campus face-to-face attendance. All study and assessment requirements are online only. Unit includes some timetabled activities, with a requirement for students to participate online at specific times. In exceptional cases (noted in the Handbook) students may be required to participate in face-to-face laboratory classes when a return to UWA’s Crawley campus becomes possible in order to be awarded a final grade.
External
No attendance or regular contact is required, and all study requirements are completed either via correspondence and/or online submission.
Off-campus
Regular attendance is not required, but student attends the institution face to face on an agreed schedule for purposes of supervision and/or instruction.
